Looking at this from a different angle. Maybe he slept in the nude out of habit..then maybe he woke up to take his prescription drug and had a heart attack and dropped the bottle spilling the pills.

you should be, they only had a few reshoots to do and most of those were actiony sequences...plus, they've already released some of the trailers...he was so much more inspired than when jack nicholson played him, he did it like he really became the character rather than just acting the character
